Instructions
Complete the identity, citizenship, contact, and address details from CS Form 212.
Supply spouse, children, father, and mother information. Enter N/A where not applicable.
List all schools from elementary to graduate studies. Click "Add Row" for each level attended. Include school name, degree/course, inclusive dates, and honors.
List all eligibility titles, board/bar exams, and special laws. Include rating, conferment date/place, and license details.
Per CS Form 212 Attachment (Annex H-2). Include only experiences relevant to the position applied to. List from most recent first.
List civic, non-government, people, or voluntary organization involvement. Click "Add Row" for each involvement.
List all training programs / L&D interventions attended. Include dates, number of hours, type, and sponsor.
List each special skill, recognition, and membership separately. Click "Add Row" for each entry.
Answer the required sworn questions and provide references and government-issued ID details.
Please confirm that all required PDS entries are complete and accurate. Once submitted, a transaction code will be generated for tracking.
